Your kids will have a blast learning new games, sports or creative activities at ActivKids, our new before and after school recreation program for kids in grades K-5.

Your kids will love ActivKids for the fun and friends. You’ll love ActivKids for knowing your children are safe, supervised and feel at home in the familiar setting of their own school. ActivKids is convenient, too. Flexible morning drop-off and evening pick-up times mean no rushing to beat the clock. Register for the whole year to match your schedule, or purchase a punch pass to use on a drop-in basis.

Punch Pass:
Good for five before or after school admissions, only $57. To use the Punch Pass for After School ActivKids, please call no later than 3:00pm on the day of use to ensure availability. No call is necessary for Before School ActivKids. Punch Passes are for occasional and flexible use.

Days Off Club

ActivKids Days Off Club

Spend your days off with ActivKids for Grades K-6! Start the day with free play, crafts, organized games and physical activity. Afternoons are specially-planned ActivTime sessions, like local fieldtrips, in-house special events or theme days. Bring a sack lunch and drink with your name clearly marked.

Join us for ActivTime only, or add extra hours in the morning or early evening.

ActivKids Days Off Club takes place at Simkus Recreation Center and is open to students of all school districts.

To register for ActivKids complete the following steps:

• If you want to register for the full school year and choose a set schedule complete and sign the ActivKids Registration Packet.
• If you want a flexible option with no set schedule, complete the Punch Pass registration form.
• If you wish to have your payments automatically deducted from your bank account each month, also complete the EZ Pay form.
• Submit the forms by email, or print and mail/fax the forms, together with your first payment, to Simkus Recreation Center..
• If you need a flexible option, complete the Punch Pass form

Payments

Monthly Payments:
The total cost of the ActivKids Before and After School Care program will be divided into nine equal monthly payments, due the first day of each month. Yous will be notified of your exact monthly payment after your child’s registration is processed. All monthly amounts due will be listed on the confirmation receipt that will be mailed to the
child’s home. The computer will automatically calculate the monthly payment based on how many days of school there are in the current year and what attendance schedule your child is registered for.

Payment Due at Registration:
Full payment due at registration includes: 1) Last Month in advance, and 2) Current Month, prorated if needed. If registration is completed before September 1, 2011, only the last month is due in advance.

EZ Pay Payment Plan Option:
Avoid late fees! Sign up for the EZ Pay payment plan option to have your credit card or your bank account debited automatically.

Third Party Billing Available:
With both parent/guardian’s permission, we can split the child(ren)’s registration fees between the two separate accounts. The child(ren) will still only be registered under a primary account, to easily track their registrations, but a second account will share the cost of the program. Both accounts can be registered for the EZ pay option. The benefit of this new feature is to provide a way to simplify payment tracking for our customers, including separate account statements and separate tax statements.

You may pay for ActivKids in one of five ways:

1.) Drop off your payment at: Simkus Recreation Center, 849 W. Lies Road, Carol Stream, IL 60188, Attn: ActivKids
2.) Mail your payments to the above location.
3.) Use the 24 hour drop box at the Simkus Recreation Center.
4.) Fax a completed payment slip with a Credit Card # to the Attention of ActivKids at 630/289-1972.
5.) Sign up for the EZ Pay plan for most programs to have your credit card or checking/savings account automatically debited on the dates due.

IMPORTANT PAYMENT INFORMATION

• Payments are NOT accepted at any of the schools – No Exceptions
• 1st late payment penalty = $20
• 2nd late payment penalty = $45
• 3rd late payment penalty = EZ Pay required to continue participation in the program
• Any late payment that does not include the late fee will be considered an incomplete payment.
• Monthly installments are based the number of school days and represent nine equal monthly payments.
• Payment amounts already include discounts for school holidays, breaks, and other non-attendance days.
• Make checks payable to the Carol Stream Park District.
• Consistent late payments and/or non-payment may result in suspension from one and/or all CSPD programs.
• Changes of enrollment can be made at your child’s Site or with the Service Team Staff.

Registration Procedure

All registrations are subject to acceptance based on program availability, proper completion of registration material, payment status and history, and the Park District’s ability to meet any special needs the participant may have.

Program maximums/minimums and locations are subject to change.

In order to register, each participant must:

• Complete and sign proper registration and emergency material for each program for each child.
• Include full proper payment prior to the program deadline.
• Pay all outstanding fees due to any and all CSPD ActivKids Programs, or any other CSPD programs and remain in Good Standing with the Carol Stream Park District.

Registration Standard

Our goal is to accommodate as many families as possible. The Carol Stream Park District reserves the right to adjust minimums/maximums and locations based on the availability of space, staff, and enrollment in order to maintain the safety, structure and integrity of the program as a whole.

• All registrations must be accompanied by full and proper payment per the ActivKids Registration Forms.
• All ActivKids Programs offer priority registration to current participants and their siblings.
  All registrations accepted during Open Registration is on a first come, first serve basis and will be given equal consideration.
• No participant is permitted to temporarily withdraw from the program.
  Any participant withdrawing from the program:
  will not be guaranteed a spot should they wish to return.
  must resubmit complete Registration and Emergency forms.
  Special needs accommodations can be made to assist your child.
