ActivKids Registration Form

How to register:

1. Complete this online registration form and emergency information form.

2. After you submit the forms a park district service team member will contact you within 48 hours to take your payment information. (Your registration will not be complete until payment is taken.)

3. If you wish to have your payments automatically deducted from your bank account each month, also complete the EZ Pay form.

Select one or both
select which days your child will be attending before care
select which days your child will be attending after care
Allow 48 hours to process. Include payment for current month. Call for a prorated amount.
