Mini Golf Party Request

This form is only a “request for availability”. A Birthday Request Form must be filled out and submitted and approved to complete the booking. Once approved, a Birthday Party Contract is generated and full payment is due within seven (7) days. It is only upon execution of the contract, approval and receipt of payment that the birthday party is guaranteed.

  • Party price includes 12 guests including the birthday child. Additional guests are $10 each but may not exceed 30 guests total for Outdoor and 16 guests total for Indoor. Extra fee does not include extra food.

Once approved, a Party Contract is generated and full payment is due at the time of registration. It is only upon execution of the contract and receipt of payment that the rental request is guaranteed.

** Additional children $10 plus extra food costs may be incurred. Two weeks’ notice is required to book parties. No outside food is permitted. Adult chaperones are required.

Do not send out party invitations until you receive an approval confirmation email. Our party packages are designed for the birthday child and 11 of their friends. If you want to invite adults and/or family to the party, inquire about our group outing and rental option. Prices are subject to change. Extra food and beverage items must be ordered through the Carol Stream Park District a minimum of seven days prior to the scheduled party. Additional Party Menu items must be paid in full at the time of order. There is no refund for extra food and beverage items. You may bring in cupcakes, a cake or prepackaged desserts. No other outside food is permitted.
