Credits, refunds, and proration are not provided for Lap Swim memberships for any reason, including due to absences of any kind or moving/relocating.

Medically-related suspensions are available and require a doctor’s note. Medical membership suspension is available from the date the suspension form is received; retroactive credits or refunds are not available. I have read and understand the cancelation policy of this contract.

Members must provide two forms of proof including driver’s license, state ID, or utility bill.

Members age 12-13 must be accompanied and supervised by a member age 18 or older.

After the first replacement membership card, there is a $5 fee for additional cards. The Carol Stream Park District app can also be used as your membership card.

Members must scan in at the front desk upon arrival.

Memberships are not transferrable and may not under any circumstance be shared. Violations result in a suspension of membership (without credit or refund) and $50 fine. Upon payment of the fine, membership privileges are reinstated. Repeated membership abuse may be subject to termination of membership.

All lap swim members are expected to comply with Carol Stream Park District code of conduct and Fountain View Fitness/Lap Swim rules and guidelines. Amenities other than the indoor pool may not be used. Violations to can result in membership/facility suspension. Repeated violations can result in fines, membership cancelation, or in extreme situations, permanent banning from all Park District facilities. No refunds will be issued for time lost due to a suspension or banning.
